Carer’s Health Check

Are you caring for someone else?

As a carer for someone else you may not always prioritise your own health care needs. Our free Carer’s Health Check is a 30-minute telephone appointment with a professional health check practitioner to discuss your own health and wellbeing needs.

 

What happens at the check?

 

During the appointment, the health check practitioner will ask some questions about your diet, weight, how much activity you do, smoking status, as well as your mental wellbeing.

 

At the end of the appointment, the practitioner will discuss how you can stay healthy and if eligible, you’ll be referred into one of our free healthy lifestyle services. You’ll also be given access to relevant local services and sources of support.

Eligibility

Resident of Surrey, work in Surrey, study in Surrey or registered with a Surrey GP

+

Over the age of 16

+

Unpaid carer

Carer’s Health Checks face-to-face

From January, we will be offering face-to-face health checks at multiple locations across Surrey in partnership with Action For Carers Surrey.

 

The face-to-face appointments are open to you if you are:

 

    • Aged between 40-74
    • Not had a health check in the last five years or being supported by a GP for ongoing health conditions.t
    • Not on medication for a long term health issues e.g. stroke, diabetes, high blood pressure
