How to Refer

One You Surrey has been commission by Surrey County Council to deliver Tier 2 smoking cessation and adult weight management programmes to residents of Surrey.

Lose Weight

If your client meets our eligibility criteria, you can refer them via the One You Surrey referral form on EMIS web or SystmOne or complete the referral form below and email it to tt1.oneyousurrey@nhs.net. Your patient can also self-refer on our homepage.

 

Criteria

 

  • Aged 16+
  • BMI greater than 30 OR greater than 27.5 for Black, Asian and Minority (BAME) community. (For MAN v FAT Challenge, BMI must be greater than 25 and for MAN v FAT Football, BMI must be greater than 27.5)

Stop Smoking

Maternity

We work closely with midwifery teams across Surrey through our stop smoking pregnancy lead, Tutua.

 

How we support pregnant smokers

 

After receiving a referral, our pregnancy lead, Tutua, will contact the patient within 48 hours. The first appointment is 30-minutes, followed up weekly 15-minute appointment. The patient will receive free stop smoking nicotine replacement and behavioural therapy. Every month we will report back to the respective midwives about the patients smoking status to ensure a high quality of care.

Mental Health

We work closely with mental health service across Surrey through our stop smoking mental health lead, Greg.

 

How we support mental health services

 

After receiving a referral, our mental health lead, Greg, will contact the patient within 48 hours. The first appointment is 30-minutes, followed up weekly 15-minute appointments. The patient will receive free stop smoking nicotine replacement and behavioural therapy. Every month we will report back to the respective mental health services about the patients smoking status to ensure a high quality and continuation of care.
