One You Surrey has been commissioned by Surrey County Council to deliver smoking cessation and adult weight management programmes.
We help you support the residents of Surrey to lead healthier lifestyles through simple and sustainable changes.
1. Using Systm1 or EMIS Web
You can easily refer using Systm1 or EMIS web. The referral goes straight to our secure nhs.net inbox and we will contact the client and triage.
2. Using our referral form
Healthcare professionals can refer by completing our referral form and emailing it to [email protected].
3. Self-referral
Patients can complete a simple self-referral form. If your patient is not yet ready to be referred and would like more information, please ask them to visit our website.
Over 12’s who are currently smoking tobacco.
Over 16’s with a BMI greater than 30 OR greater than 27.5 from a Black, Asian and Minority (BAME) community.
We work closely with midwifery teams across Surrey through our stop smoking pregnancy lead, Tutua.
How we support pregnant smokers
After receiving a referral, our pregnancy lead, Tutua, will contact the patient within 48 hours. The first appointment is 30-minutes, followed up weekly 15-minute appointment. The patient will receive free stop smoking nicotine replacement and behavioural therapy. Every month we will report back to the respective midwives about the patient’s smoking status to ensure a high quality of care.

How we support mental health services
After receiving a referral, our mental health lead, Greg, will contact the patient within 48 hours. The first appointment is 30-minutes, followed up weekly 15-minute appointments. The patient will receive free stop smoking nicotine replacement and behavioural therapy. Every month we will report back to the respective mental health services about the patients smoking status to ensure a high quality and continuation of care.
