HOW TO REFER
1. YOUR CLINICAL OPERATING SYSTEM
Refer through your clinical operating system, eg EMIS/SYSTEM ONE. The referral goes straight to our secure NHS.net inbox and we will contact your patient within 72 hours and triage.
2. SUBMIT A REFERRAL
Head to Get started and submit a referral on behalf of your patient by answering a few quick questions and our team will contact your patient and triage appropriately.
3. Self-referral
All patients can self-refer through the
Get started button on our website. They will answer a few quick questions and based on their responses we will contact them and triage onto the programme that’s right for them.
Eligibility criteria
The criteria vary a little depending on the programme (see below), however, to join our programmes, your patient must be:
- A resident of Surrey or registered with a GP in Surrey. Or working in Surrey (smoking cessation).
- Motivated and ready to make a change.
ADULT WEIGHT MANAGEMENT
- Aged 18 or over.
- BMI over 30 (over 27.5 if South Asian, black and minority ethnic groups).
- BMI reading must have been recorded within the last 12 months of referral.
Exclusions
- Pregnant or breastfeeding.
- Eating disorder.
- Accessed the service within the last 3 months.
- Have an underlying medical cause for obesity and would benefit from more intensive clinical management than a tier 2 service.
- Have significant co-morbidity or complex needs as identified by their GP or other Healthcare professionals.
- Referred to a tier 2 weight management service by a health professional in the last 6 months.
SMOKING CESSATION
- Aged 12+.
- Using or smoking tobacco.
