HOW TO REFER
1. YOUR CLINICAL OPERATING SYSTEM
You can easily refer through your clinical operating system e.g. Systm One or EMIS Web. The referral goes straight to our secure NHS.net inbox and our team will contact your patient and triage within 2 working days.
2. SUBMIT A REFERRAL
You can submit a referral on behalf of your patient, and our team will contact them directly. Our professional referral form is quick and easy to complete.
3. Self-referral
All patients can self-refer by clicking the
Get started button on our website. It’s quick and simple, and we’ll guide them step-by-step to find the perfect programme.
Eligibility criteria
The criteria vary a little depending on the programme (see below), however, to join our programmes, your patient must:
- Live or be registered with a GP in Surrey.
- Motivated and ready to make a change.
ADULT WEIGHT MANAGEMENT
- Aged 18 or over.
- BMI over 30 (over 27.5 if South Asian, black and minority ethnic groups).
Exclusions
- Eating disorder.
- Pregnant or breastfeeding.
SMOKING CESSATION
- Aged 12+.
- Using or smoking tobacco (e.g. cigarettes/shisha).
- Live, work or be registered with a GP in Surrey.

ONLINE TRAINING
We offer training for health, care and community professionals making it simple to gain the knowledge needed to better support the residents of Surrey. We’ll support you to feel more confident when discussing weight management, smoking cessation and other difficult lifestyle changes.
MARKETING MATERIALS

OUTREACH & EVENTS
Do you have an event you’d like One You Surrey to attend? Our Community Outreach and Inclusion Officer can attend community events and health fairs, deliver short talks or presentations, and engage directly with staff teams and service users.
PRIMARY CARE
Our dedicated Primary Care Liaison Officer can support practices by generating referrals from patient lists, assisting with the coding of responses, and attending PCN or practice meetings to provide advice and guidance, helping to improve engagement with the One You Surrey smoking cessation service.
